School Health Questionnaire
If your child is starting primary school, you will have received an information pack with a letter, a leaflet about our School Health Service and a school health questionnaire.
It’s important that you complete and return the questionnaire so that we have the most up-to-date demographic and health information about your child.
It lets the school nurse know if they need to be aware of any essential medical information about your child. This is especially important if they require any medication to be administered in school.
The information you provide is confidential and will only be shared on a need-to-know basis with the school to make sure that your child is safe and receives appropriate care so they can stay safe and well at school, and achieve their potential.
If you have misplaced your school health questionnaire, or you need another copy, you can download it below and print it out. Once you have completed it, please hand this in to the main reception at your child’s school and our school nurse will collect it.
